Whether you are a member, family member, friend, or community supporter, you may use this form to share a birth announcement, notify us of the passing of an immediate or close family member, or submit an Above & Beyond nomination recognizing a member’s exceptional service.

Because we are not always aware of personal milestones or losses as they occur, your submission helps ensure our League can celebrate, support, and recognize our members in a timely and thoughtful way.
